How does the face change over time?

As you age, you get hollowing of the temples causing the brows to drop. The fat pads under the eyes reduce resulting in formation of tear troughs or under eye hollows. You lose fat at the back of the face around the cheeks which causes the skin to fall forwards giving rise to nasolabial folds, marionette lines, downturned mouth corners and jowls. All of this results in squaring of the face. Hence you start looking tired, sad and exhausted.

So rather than using dermal fillers to chase lines on the face such as the nose to mouth or mouth to chin lines, you will get a far superior result if the filler is placed strategically in areas of volume loss. This will help “pull” the skin back giving a more youthful appearance. Whereas just filling up the lines gives an “over-filled” or cushion appearance.

What are dermal fillers and how can they help?

Dermal filler is a gel like substance containing hyaluronic acid. This acid is also present in skin and is responsible for retaining moisture and keeping the skin plump. By adding hyaluronic acid to the skin, dermal filler add volume by attracting and holding on to moisture. This results in hydrated skin and its plumpness is enhanced. Dermal fillers also stimulate collagen production over several weeks thus strengthening the skin. The main brands of fillers are Juvederm, Restylane, Belotero and Teosyal. Most fillers contain lidocaine which help reduce discomfort during injections.

Juvederm have brought out a premium range of fillers with Vycross technology. This allows cross-linking of lower molecular weight hyaluronic acid molecules resulting in a smoother product that integrate better with the skin. In practice this means that the product is easier to inject, it is less painful, has a greater lifting potential and because it blends well with skin, the result is more natural in look and feel.

At Luxe Skin we only use the Juvederm premium fillers called Volbella, Volift and Voluma. Volbella is a light filler suitable for under the eyes, forehead and upper lip area. Volift is a medium strength filler used all over the face for volume and lift. Voluma is the highest strength filler that is used in areas which need the most volume and lift such as the cheeks and jaw.

What are the benefits of dermal fillers?

Dermal fillers provide a fast and effective method of rejuvenating the skin. The procedure is minimally invasive with little downtime. The results are visible instantly and settle after 2 weeks. They continue to improve for 3 months due to collagen stimulation. Fillers can correct lines, add volume, enhance facial features and also help in lifting the face. They are great at improving the ageing skin and give it a youthful plumpness. Hyaluronic acid fillers are reversible as they can be dissolved with an enzyme called hyaluronidase (Hyalase) which turns the filler into water. Hence most of the complications can be easily resolved in this way.

The main treatment areas for dermal fillers are:

  • Cheeks
  • Under eye hollows or tear troughs
  • Nose to mouth lines
  • Smokers lines on upper lip
  • Lip volume
  • Jaw contouring
  • Chin contouring
  • Nose reshaping
  • Liquid facelift by adding filler in strategic points thus lifting the face

What is liquid facelift

Liquid facelift is full face rejuvenation using 8-12 syringes of filler placed carefully around the face rather than just aggregating them in the lines. This results in a natural, contoured and younger looking YOU. We use Juvederm Voluma and Volift which lasts 18 – 24 months. So after your initial treatment, you only require small amount of regular maintenance to combat the signs of ageing as they happen. It is best to start treatment sooner than later. As the longer you leave it, the most expensive it becomes to have the desired result.

What are the side effects of dermal fillers?

Minor side effects are bruising, redness and swelling which usually resolves within 2 weeks. The filler can sometimes migrate away from the site of injection into the adjacent area resulting in a displeasing aesthetic look. It is easily corrected by dissolving the filler and repeating the initial procedure. If the filler comes up to the surface of the skin, it may present as a swelling that does not settle after 2 weeks. Occasionally it may give a blueish hue as the light hits it. This is called the Tyndall effect. It can be resolved by dissolving the filler that has surfaced.

Dermal filler can aggregate and lump which requires an injection of hyalase, an enzyme that breaks the filler and turns it to water. Rarely the filler may form a hard lump that does not respond to hyalase. Such a lump is called a granuloma which requires a steroid injection to break it. A much rarer problem with fillers is the formation of a biofilm. The filler can get infected and harbour bacteria. Luckily this problem is rare but it does require treatment. In such a case the filler will need to be dissolved and oral antibiotics may be needed for several weeks.

So how can the complications be avoided or minimised? Firstly, go to a reputable clinic where the treatment is being carried out by an experienced medical professional. Do not go to a cut price clinic. The reason they are selling treatments cheap is because they have not spent much time or money on their own training and education. They are not using high quality products and they won’t be able to deal with complications if they occur.

  • I, Doctor Q have spent a lot on my training so you can be confident that I use the latest techniques in administering dermal fillers.
  • I do not inject filler in the most dangerous area which is the glabella – the 11 lines between the eyebrow. I feel wrinkle relaxing injections are best suited here.
  • I use a blunt cannula in high risk areas such as the tear trough
  • I always pull back the plunger of the syringe to ensure I am not in a blood vessel
  • I inject slowly to reduce risk of pushing filler in vessels
  • I always keep emergency supplies to use in case a problem occurs
  • I disinfect the injection site to reduce infection risk
  • I use the best quality fillers that integrate well with the skin

What is the downtime with dermal fillers?

Any swelling and bruising usually settles within 2 weeks. The filler has also integrated with the skin by the time.

How long do dermal fillers last?

Results last 9-24 months. The longevity depends on how fast your body breaks it down and the amount of filler used.

How much do dermal fillers cost?

Fillers are usually priced per 1ml syringe. At Luxe Skin we only use the premium Juvederm Vycross range of fillers. 1ml is priced at £375.

Liquid Facelift v Surgical Facelift?

A surgical facelift only addresses one concerns which is loose skin. It does not make the face more contoured or address volume loss around the cheeks, eyes and temples. Also it is a high risk procedure with complications associated with surgery and anaesthesia. If the surgery results in any facial asymmetry, it can be difficult to correct and requires repeat surgery. The downtime after the surgery can be 4-6 weeks. It costs £6k – £8k and results last around 5 years. It can often give a “stretched” appearance to the face.

  • Full face rejuvenation using Juvederm has the following advantages
  • Literally no downtime. You can go back to work the next day
  • Much smaller risk profile than surgery
  • Treatment can be easily “topped-up” and corrected in case of asymmetry
  • Fillers are reversible and can be dissolved using Hyalase. Hence complications areeasier to correct.
  • Addresses the cause of sagginess by replacing volume loss giving a more natural appearance
  • Easier to maintain with small regular treatments targeting age effects on time
  • Treatment is tailored so you look less tired and saggy and more contoured and younger
  • People often say it makes them look 10 years younger

Dermal Fillers vs. Threads

Dermal fillers are best for skin rejuvenation. They add volume, strength the skin, reduce lines and folds and lift the face to some extent. Choose fillers is volume, line reduction and skin youthfulness is important to you.

Threads have the most lifting potential. They are best for lifting jowls and mid face. Threads also stimulate collagen and tighten the skin. Choose threads if a facelift is most important to you.

For best results, we at Luxe Skin combine threads and fillers for the ultimate facelift.
